目的 对比应用新型再生可降解生物材料猪小肠黏膜下层脱细胞修复补片(SIS)与植皮术在修复手部软组织缺损的治疗效果.方法 2017年12月至2018年12月,共收治手部软组织缺损36例,根据缺损面积与治疗方法分为两组:补片组21例,软组织缺损面积2.0 cm×1.5 cm^9.0 cm×3.5 cm,平均5.3 cm×2.1 cm,采用SIS治疗;植皮组15例,软组织缺损面积9.0 cm×4.0 cm^16.0 cm×9.0 cm,平均12.0 cm×8.5 cm,采用中厚皮片植皮治疗.观察两组治疗方法促进软组织缺损愈合的效果,记录术后14 d、21 d、28 d、3个月创面区愈合情况,并随访评估创面区愈合后的外观、色泽、弹性、感觉恢复与部分肌腱外露的治疗效果.结果 本组36例均获随访,随访时间3~10个月,平均5个月.两组创面均完全愈合,外观、色泽接近,皮肤弹性及感觉均恢复良好.补片组感觉恢复优14例(66.6%),良5例(23.8%),差2例(9.6%);植皮组感觉恢复优9例(60.0%),良4例(26.0%),差2例(14.0%).创面愈合效果补片组优14例,良5例,差2例;植皮组愈合优9例,良4例,差2例.结论 SIS能快速、有效的刺激机体再生出表皮组织,并且新生的表皮经过生长与周围皮肤颜色无明显差异,无明显瘢痕增生,是一种手部小面积浅表软组织缺损的理想修复材料.  相似文献

目的:探讨丹参注射液对带蒂皮瓣移植修复术后成活率及预后的影响。方法:将68例接受带蒂皮瓣移植修复术的患者简单随机分成观察组与对照组各34例。对照组给予术后常规治疗,观察组在此基础上加用丹参注射液治疗,持续给药1周。随访3~12个月,比较两组患者术后皮瓣成活率、皮瓣肿胀程度、凝血指标凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)水平变化及并发症发生率。结果:观察组患者皮瓣成活率为97.06%,明显高于对照组的76.47%,差异有统计学意义(P<0.05)。观察组术后7 d皮瓣肿胀程度明显轻于对照组,差异有统计学意义(P<0.05)。观察组患者术后3 d的PT、APTT、FIB水平[(17.84±2.45)s、(35.38±3.14)s、(2.24±0.17)g/L]均明显低于对照组患者[(23.07±2.69)s、(41.56±3.34)s、(2.79±0.32)g/L],差异有统计学意义(P<0.05)。观察组患者术后伤口感染、瓣下积液、动静脉危象的总并发症率为8.82%,明显低于对照组的29.41%,差异有统计学意义(P<0.05)。结论:丹参注射液能提高带蒂皮瓣移植修复术后成活率,减轻术后皮瓣肿胀程度,改善凝血指标,降低血管危象、皮瓣坏死等并发症风险。  相似文献

BackgroundElectrochemotherapy combines electroporation in conjunction with chemotherapeutic agents and is used to treat tumours in many localisations, including cutaneous metastases. The symptoms associated with cutaneous malignant wounds can be distressing for patients and their management is a challenge in healthcare.AimThe purpose of this systematic review was to investigate the effectiveness of electrochemotherapy in the context of palliative care.DesignAll aspects of the systematic review were followed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement.Data sourcesThe following databases were searched for English-language reviews; Medline, Embase, CINAHL, British Nursing Index and the Cochrane Library. The search was conducted between the publication of Standard Operating Procedures in 2006 and the third week of October 2017. Studies involving oral cancers and studies with fewer than 10 patients were excluded. The selected studies were assessed for risk of bias and sub-group data were synthesised in a random-effects meta-analysis.ResultsFrom 425 studies, 29 studies were included involving 1503 patients, the pooled results were 46.6% for complete response and 82.2% for objective response according to the Response Evaluation Criteria in Solid Tumours. The meta-analysis indicated that small tumours were over twice as likely (2.25) to have a complete response than large.ConclusionsElectrochemotherapy is an effective, repeatable and minimally invasive intervention within the palliative population that can reduce symptom burden. This review is an update of previous systematic reviews by Mali et al. [1,2] and highlights the need for tailored treatment depending on each individual case.  相似文献

BACKGROUNDReconstructive repair of huge full-thickness abdominal wall defects following debridement for abdominal electric burns remains a clinically challenging task. An ideal abdominal wall repair means a re-closure of the defected abdominal wall with pedicled neurovascular myofascial flaps, restoration of the abdominal wall integrity, and maintenance of the abdominal wall muscle tension to prevent the occurrence of abdominal wall hernia. When treating huge full-thickness defects, composite autologous tissue flaps are a good option for the repair. CASE SUMMARYThis study reported the case of a 43-year-old male patient suffering from full-thickness abdominal wall defects complicated with necrosis of multiple bowel segments and duodenal leak following high-voltage burns involving the left upper limb and abdomen. After debridement for abdominal electric burns and end-to-end anastomosis for the necrotic bowels, reconstruction with acellular dermal matrix grafting and vacuum sealing drainage were performed for temporary abdominal closure. The remaining 18 cm × 15 cm full-thickness abdominal wall defect was repaired using a combined anterolateral thigh and tensor fascia lata free flap. The proposed method achieved the functional reconstruction of the abdominal wall.CONCLUSIONThis approach restored the abdominal wall integrity, maintained certain muscle tension, avoided abdominal hernia, reached satisfactory aesthetic effect, and resulted in no complications in the grafting regions.  相似文献

Free microsurgical tissue transfer of the latissimus dorsi flap may be indicated for the restoration of intra- and extraoral defects, especially when a large-sized skin island flap is required. In many cases, use of the latissimus dorsi flap for coverage of large-sized intraoral defects results in bulkiness due to the proportion of subcutaneous fat. Prelamination of free flaps appears to be a promising technique to overcome this flap bulkiness. This modification in flap design could improve the postoperative functional outcome, as well as reduce donor site morbidity. This article presents four novel clinical cases, in which the patients underwent prelamination of the latissimus dorsi flap with local skin grafts during oral cancer treatment in order to reduce the thickness of the free flap and allow tension-free primary closure of the donor site. These attempts successfully covered large-sized intraoral defects, achieving good functional outcomes with minimal donor site morbidity.  相似文献

BackgroundExtensor hallucis longus (EHL) tendon injuries often occur in the setting of lacerations to the dorsum of the foot. End-to-end repair is advocated in acute lacerations, or in chronic cases when the tendon edges are suitable for tension free repair. Reconstruction with allograft or autograft is advocated for cases not amenable to a primary direct repair. This is often seen in cases with tendon retraction and more commonly in the chronic setting. In many countries the use of allograft is very limited or unavailable making reconstruction with autograft and tendon transfers the primary choice of treatment. Tendon diameter mismatch and diminished resistance are common issues in other previously described tendon transfers.MethodsWe present the results of a new technique for reconstruction of non-reparable EHL lacerations in three patients using a dynamic double loop transfer of the extensor digitorum longus (EDL) of the second toe that addresses these issues.ResultsAt one-year follow up, all patients recovered active/passive hallux extension with good functional (AOFAS Score) and satisfaction results. No reruptures or other complications were reported in this group of patients. No second toe deformities or dysfunction were reported.ConclusionsSecond EDL-to-EHL Double Loop Transfer for Extensor Hallucis Longus reconstruction is a safe, reproducible and low-cost technique to address EHL ruptures when primary repair is not possible.Level of evidenceIV (Case Series).  相似文献
